1. Transitions of the Genitals
By transitions of the genitals Hirschfeld understood people whose genitals reveal features of both sexes. They were called hermaphrodites.

In our society, every person has to have one(!) sex. Hirschfeld drew up expert reports on cases in which the assignment to one sex proved difficult. For such reports, it was crucial to determine: 1. which sex the person concerned wanted to belong to, and 2. whether the "male" or the "female" physical and psychical content predominated.

In his report on the photographed 14-year-old Bertha D., Hirschfeld summed up as follows:

"A. Of a completely  m a l e   n a t u r e   are: beard growth, body hair, pubic hair, voice, and-in a negative respect-the breasts, in particular.
M o r e  m a l e   are: penis, facial expressions, pelvis, bodily movements. Of the male sexual features which could not be established are: testicles, scrotum, prostate and ejaculation.
B. Of  a  f e m a l e   n a t u r e   are: the labia majora (the large pudendal lips).
M o r e   f e m a l e   are: urethra and vagina. Of the female sexual features which could not be established are: ovaries, uterus, Fallopian tubes and menstruation.
C. A l l   o t h e r   p h y s i c a l   a n d   p s y c h i c a l   f e a t u r e   a r e   m i x e d.
D. The sexual drive is   n o t   at all present, i.e. neither male nor female. To date, any distinct sexual awareness - be it male or female - has been lacking.
"
Hirschfeld concluded his report: "... Bertha D, as she has been so far, [is] to be assigned to the male sex. Her sex has to be changed accordingly to male, and her name, Bertha- at her own request- has to be changed to Berthold in the register of births, deaths and marriages... Dress and hairstyle as well as identity papers are likewise to be altered. Her   w i s h   t o   b e   b a p t i z e d   a n d   c o n f i r m e d   a s   a   b o y   is well-founded. 3
